GPs are often asked to complete a variety of forms for patients, such as for the Benefits Agency or for Life Assurance. Requests for this service should be made in the first instance to reception. Same day or pre-booked appointments should not be used to make such requests.
Completion of forms and letter will take a minimum of three days to process.
Under NHS legislation, a fee will be charged for the completion of certain forms and reports. In these circumstances you will be advised of the likely fee beforehand and should you subsequently agree to this fee it will apply, even if you then change your mind, should we have already completed the work requested.
Under these circumstances we may pursue you for any non payment.
