Advanced Nurse Practitioners are Registered Nurses who have completed additional training and academic qualifications to be able to examine, assess, diagnose, treat, prescribe and make referrals for patients who present with undiagnosed/undifferentiated problems. Leah also has a special interest in minor ops.

PCN

First Contact Physiotherapist (FCP)

Our in-house FCP operates at an advanced level of clinical practice with skills to assess, diagnose, treat and manage musculoskeletal (MSK) problems and undifferentiated conditions. Patients are referred on to our FCP as a result of clinical presentation via eConsult, the practice website, telephone triage or after assessment by a member of the surgery wider clinical team.

Social Prescribing

Our social prescribers work with patients who would like support to access services and activities in the community to improve their health and wellbeing. Patients can be referred for a consultation with our in-house social prescriber or self-refer.

Community ANP

Working as an integral member of the surgery team to support housebound patients and patients in Care and Residential homes.

Pharmacy Team

Consisting of clinical Pharmacists and Pharmacy Technicians working to improve medication safety and management throughout PCN practices.

Mental Health

We now have access to developing mental health services within the PCN.


Healthcare Team

Integrated Diabetes Care

Monthly joint patient clinics run by our in house nursing team and a diabetic specialist nurse from University Hospital Plymouth. Complemented by regular virtual clinics held with our in house diabetic clinical team and Paddy English (Diabetic Consultant from University Hospital Plymouth).

Midwife

Providing care during and immediately after pregnancy at the surgery, local children’s centre (Four Woods) or at home.

Health Visitors

Provides health advice to adults and monitor child development. Health visitors are based at the local children’s center (Four Woods).

MacMillan Nurse

Providing palliative care in the community.

Community Nurses

The district nursing team are available to help housebound patients with their healthcare.

Counselling (MIND)

This service is provided in Plymouth by self-refer.
