Comprehensive management of the treatment of a range of chronic diseases (diabetes, heart disease, COPD, asthma, etc) is provided by the GPs and nursing team. There are multiple forms in the health review and assessment clinical room.
We would request that you complete this at least once a year in the month of your birth so that we may monitor your progress and arrange further follow up as required. Medication reviews will also be moving to the birth month as well.
